Who prepares the Care Plan?

In most instances, the Care Plan is a joint effort between Golden Home Care, the person receiving care and their family or next of kin. In the event that the individual is not able to take part in the preparation of the Care Plan due to Dementia or Alzheimer's, we will consult directly with his or her next of kin and other healthcare professionals. Our caregivers take time to read the Care Plan that has been drawn up and to follow it accordingly.

Our Care Plans are reviewed every three to six months to make sure that they allow for complete and tailored care.

Having a Care Plan will likely reduce hospitalisations and trips to the emergency rooms. The Care Plan can include information that prevents accidents, such as issues with safety in their homes, risk assessments, falls risks, and mobility concerns.
