Understanding the Client Care Plan and Who Prepares it
When you first meet with the Home Care assessor from Golden Home Care, they will undoubtedly fill you in regarding the importance of having a Client Care Plan in place. But what is a Client Care Plan? Why is it such an important document? And who is responsible for preparing it? We answer all of your questions below.
What is a Client Care Plan?
A client care plan is a comprehensive document that details a safe and effective routine for the individual in question based on their personal preferences, beliefs, and unique situation.
The plan essentially outlines how our live-in caregiver will look after your senior loved one on a daily basis. It will highlight routines and daily practices revolving around hygiene, nutrition, sleep, exercise and mobility, and oral care.
More often than not, the plan will also include information on how the Care assistant needs to communicate with the Client and help to look after their emotional and psychological needs. Fall prevention is another important aspect of most Client Care Plans.
Why is the Client Care Plan so important?
A Client Care Plan is a legal document between Golden Home Care and the individual who will be receiving care.
The plan helps to ensure that our Client receives the same standard of care regardless of who from Golden Home Care is caring for them at any given time. It also ensures accurate ongoing records of the care received by the individual and supports the individual and agency in identifying, analysing, and solving any problems or challenges along the way.
Who prepares the Care Plan?
In most instances, the Care Plan is a joint effort between Golden Home Care and the individual receiving care. In the event that the individual is not able to take part in the preparation of the plan due to dementia or Alzheimer's, we will consult directly with his or her next of kin. It is then up to the Carer to ensure that he or she takes the time to read the Care Plan that has been drawn up, and to follow it accordingly.
Our Care Plan's are updated every three to six months to make sure that it allows for complete and tailored care. Should any changes be required in between revisions, it is the elderly care provider's responsibility to report this to Golden Home Care and ensure that the proper alterations are made timeously.
